Tesamorelin is a synthetic polypeptide analogous to growth hormone-releasing hormone (GHRH). It is being researched for its potential to stimulate the production and secretion of growth hormone (GH) and subsequently insulin-like growth factor-1 (IGF-1). The peptide is being primarily studied for its potential to reduce excess abdominal and visceral fat in models of lipodystrophy.
1. Mechanisms of Action 🧪
Tesamorelin's potential actions are centered on the endocrine system:
GHRH Receptor Interaction: The peptide is believed to interact with and activate GHRH receptors on the anterior pituitary gland. This interaction is thought to trigger a signaling cascade that may lead to the release of GH from somatotroph cells.
GH and IGF-1 Increase: Research indicates that Tesamorelin may increase overall GH levels by an estimated 69% and IGF-1 levels by up to 122%.
Lipolytic Action: While GH is believed to have lipolytic properties, its direct mechanism in fat breakdown is not fully understood. Tesamorelin's action on GH is thought to potentially contribute to the breakdown of fat, especially in visceral adipose tissue.
2. Research and Clinical Studies 🔬
Lipodystrophy and Fat Reduction
Visceral Fat: In two Phase III studies on models with lipodystrophy, Tesamorelin appeared to significantly decrease visceral adipose tissue levels by at least 15.4% after 26 weeks.
Liver Fat: In a study on HIV-positive models with high hepatic fat fraction (HFF), Tesamorelin appeared to reduce HFF in 35% of the subjects after 12 months, compared to only 4% in the placebo group.
Muscle Tissue: A study using CT scans suggested that Tesamorelin may be associated with improvements in muscle density and volume, with noticeable changes in specific muscle groups such as the rectus abdominis and psoas major.
Other Research Areas
Cognition: A clinical study is currently underway to determine Tesamorelin's potential effect on neurological function and cognitive performance in immunodeficient models with mild cognitive impairment.
Metabolic Markers: In studies on models with lipodystrophy, Tesamorelin appeared to decrease triglyceride and cholesterol levels. However, a separate study on models with type 2 diabetes found no significant reduction in fasting glucose, glycosylated hemoglobin, or diabetes control.
